- To provide you with counseling treatment and services and or to coordinate or discuss your treatment with other treating medical or mental health professionals.
- Professionals who work for Palo Alto Therapy may share information about your situation among each other for the purpose of diagnosis, treatment planning, and/or counselor growth.
- At times, your particular situation or course of therapy may be discussed with professional colleagues outside of Palo Alto Therapy for the purpose of clinical consultation and/or training (Your name will not be disclosed during these discussions). This is done in a respectful manner to ensure you are receiving quality care and to assist in the learning and growing of your counselor.
- To bill and receive payment for the services you receive, to submit insurance claims on your behalf, and to release billing records/scheduling information to anyone who is paying for your therapy (Insurance companies may have access to all your information including clinical outcome measures).
- For functions necessary to run our practice and to assure that all patients receive quality care.
- As required or permitted by law.
- If you initiate a complaint/litigation we may need to use information in your file to respond to the complaint. However, we will strive to release only the relevant information.
In certain situations, we may disclose your medical information without your authorization, including:
- For worker’s compensation or similar programs.
- For public health activities (e.g. reporting abuse or reactions to medications).
- To a health oversight agency, such as the California Department of Health Services.
- In response to a court or administrative order, subpoena, warrant or similar process.
- To law enforcement officials in certain limited circumstances.
- To a coroner, medical examiner or funeral director.
